ExpandMD does the actual monitoring (watching the data and calling the patients) in an RPM program.

RPM is reimbursed by Medicare for ALL patients who have a chronic condition that is monitorable. The
key conditions are hypertension, diabetes, congestive heart failure (weight scales), COPD and other
lung/breathing issues (pulse oximetery). Patients can be monitored for multiple conditions (diabetes and
hypertension, for example) but only bill Medicare for one service.
ExpandMD provides the monitoring services. ExpandMD has bilingual technicians and monitoring staff
available (ExpandMD is among the largest RPM companies in the US and have patients in parts of the
country where Spanish as a first language is common) and can assign the patients for monitoring based
on their language preference. If a practice has a need for other languages, ExpandMD can source in any
language needed.

Practice is only billed for patients that Medicare reimburses. Risk-free.
The practice is billed by ExpandMD only after Medicare has paid the practice. Cash

Monitoring staff works as an extension of the practice. The Practice decides how they
want escalation communicated and the protocol for the escalations.
The adverse trends, readings, etc. can be defined by the practice down to the individual
patient.
Monitoring staff speaks English, Spanish, Korean, Vietnamese, Mandarin, and Russian,
with other languages available if needed.
The codes involved are 99453, one time set-up, 99454, provision of the equipment, 99457 for the first
20 minutes of monitoring and 99458 for each additional 20 minutes of monitoring.
99453 - Initial set-up One time reimbursement.
99454 - Provision of the equipment Patients receive equipment and supply refills.
99457 - First 20 minutes of monitoring The ExpandMD staff reviews the data and follows protocols set.
Monitoring any actions needed when reviewing data, such as
notifications of variances, calling the patient, reminding the
patient to test, etc.
99458 - Each additional 20 minutes of
monitoring
ExpandMD charges a flat rate per patient and does not charge any additional fees for the additional
monitoring. That billing simply improves the ROI to the practice.
Compensation
For the Doctor
On average, a doctor with a Medicare patients base can add around $100,000 of bottom-line income per
year without any capital investment, no risk, without seeing more patients, disrupting or hiring office
staff, etc.
For the Referring Rep
On average, a rep will make about $1,000 in commissions per month per doctor. The commission is paid
as long as that doctor works with ExpandMD.
(10 doctors x $1,000 could equal $10,000 per month in “mail box money”)
ExpandMD Cognitive Testing
Cognition can be generally defined as the state or processes of being able to perceive and judge
reality effectively. Cognitive tests are designed to determine whether you may have any issues
with cognition, also known as cognitive impairment. These tests don't diagnose cognitive
problems. Instead, they help your provider determine if you need to take more tests or if there
are any cognitive issues needed to be address.
Older people will routinely receive cognitive tests during wellness checkups because they're
more at risk for conditions that cause cognitive impairments. Early detection can be vital in
treating or slowing down many of the causal conditions, so older adults should get tested
regularly. This is especially true for those with dementia and Alzheimer’s.
People of all ages can receive cognitive testing though, and not all symptoms and conditions
that lead to cognitive impairment are caused by age.
The challenge with Cognitive Testing is the time required to supervise the test, usually around 30
minutes. Practice don’t have the staff to spare to monitor the test. ExpandMD monitors the tests for the
practices.
